I am requesting to participate in boat rental operated by [Business Name] ("the Company"). I understand that I will operate the vessel without a Company captain aboard and that I am responsible for the vessel, its passengers, and its safe operation.

I confirm that my participation is entirely voluntary, that no one has pressured me to take part, and that I may stop participating at any time if I believe conditions are unsafe or beyond my ability.

I understand that boat rental involves inherent risks that cannot be eliminated regardless of the care taken by the Company, and that those risks include, but are not limited to:

•Collision with other vessels, docks, navigational markers, and fixed objects

•Grounding, prop strikes, and damage from submerged or unmarked hazards

•Falls overboard, drowning, and propeller injury

•Carbon monoxide exposure near the engine and swim platform

•Rapidly changing weather, wind, waves, current, and visibility

•Mechanical failure, running out of fuel, and the cost of towing

•Sunburn, dehydration, heat exhaustion, and fatigue

•Serious bodily injury, permanent disability, paralysis, and death

I have read this list, I understand it, and I knowingly and voluntarily accept every one of these risks, including risks that may arise from the negligence of others or that are not specifically listed above.

I confirm that I have accurately represented my swimming ability and the swimming ability of any minor in my care.

I agree to wear a properly fitted, Coast Guard-approved personal flotation device whenever the Company requires it, and I understand that a flotation device does not guarantee safety in the water.

I agree to follow all posted rules, safety briefings, and instructions given by [Business Name] staff, including:

•Only the named operator listed below may operate the vessel

•Coast Guard-approved life jackets aboard for every passenger and worn by children

•Staying within the permitted operating area and returning by the agreed time

•No operating the vessel under the influence of alcohol or drugs

•Observing no-wake zones, posted speed limits, and navigation rules

•Reporting grounding, collision, or damage to the Company immediately

I understand that failing to follow the rules may result in my removal from the activity without a refund.

I agree to inspect any equipment provided to me before use and to immediately report anything that appears damaged, worn, or unsafe. If I choose to use the equipment, I am confirming that I accept it in its current condition.

I accept responsibility for the reasonable cost of repair or replacement of equipment that is lost, stolen, or damaged beyond normal wear while it is in my care.

I confirm that I am not under the influence of alcohol, cannabis, illegal drugs, or any medication that impairs judgment, coordination, or reaction time, and I will not become so during the activity.

I understand the Company may refuse or end my participation, without refund, if staff believe I am impaired.

I confirm that I am physically able to take part in this activity and that I have disclosed any medical condition, injury, allergy, pregnancy, or medication that could affect my safety or the safety of others.

In the event of an injury or medical emergency, I authorize the Company to arrange first aid, emergency transport, and medical treatment on my behalf, and I accept financial responsibility for the cost of that care.

I understand that weather, water levels, wind, visibility, and other natural conditions can change quickly and may make the activity more difficult or hazardous.

I agree that the Company may delay, shorten, reroute, or cancel the activity for safety reasons, and I accept the Company's published cancellation and refund policy for those situations.
