    2. Activity and Voluntary Participation

    I am requesting to participate in zipline and aerial adventure activities operated by [Business Name] ("the Company"). I understand this activity takes place at height, that I will be secured by a harness system, and that I must actively follow launching and braking instructions.

    I confirm that my participation is entirely voluntary, that no one has pressured me to take part, and that I may stop participating at any time if I believe conditions are unsafe or beyond my ability.

    3. Assumption of Risk

    I understand that zipline and aerial adventure activities involves inherent risks that cannot be eliminated regardless of the care taken by the Company, and that those risks include, but are not limited to:

    Falls from height, including during transitions between platforms

    Harness, helmet, trolley, cable, or braking system failure

    Impact with the braking system, platforms, trees, or terrain at the landing

    Stalling mid-line and requiring a staff-assisted rescue at height

    Rope burn, pinching, and bruising from cables, straps, and hardware

    Panic, vertigo, or a fear response while suspended at height

    Weather conditions including wind, lightning, and rain during the course

    Serious bodily injury, permanent disability, paralysis, and death

    I have read this list, I understand it, and I knowingly and voluntarily accept every one of these risks, including risks that may arise from the negligence of others or that are not specifically listed above.

    4. Rules and Instructions

    I agree to follow all posted rules, safety briefings, and instructions given by [Business Name] staff, including:

    Never touching the cable, trolley, or brake unless instructed

    Maintaining the body position demonstrated during the safety briefing

    Confirming you are within the posted minimum and maximum weight limits

    Keeping helmet and harness fastened as fitted by staff for the whole course

    Launching only when the guide gives a clear signal

    I understand that failing to follow the rules may result in my removal from the activity without a refund.

    5. Equipment

    I agree to inspect any equipment provided to me before use and to immediately report anything that appears damaged, worn, or unsafe. If I choose to use the equipment, I am confirming that I accept it in its current condition.

    I accept responsibility for the reasonable cost of repair or replacement of equipment that is lost, stolen, or damaged beyond normal wear while it is in my care.

    6. Alcohol, Drugs, and Fitness to Participate

    I confirm that I am not under the influence of alcohol, cannabis, illegal drugs, or any medication that impairs judgment, coordination, or reaction time, and I will not become so during the activity.

    I understand the Company may refuse or end my participation, without refund, if staff believe I am impaired.

    7. Health, Fitness, and Medical Authorization

    I confirm that I am physically able to take part in this activity and that I have disclosed any medical condition, injury, allergy, pregnancy, or medication that could affect my safety or the safety of others.

    8. Weather and Conditions

    I agree that the Company may delay, shorten, reroute, or cancel the activity for safety reasons, and I accept the Company's published cancellation and refund policy for those situations.

    9. Release of Liability and Indemnification

    To the fullest extent permitted by the laws of [State], I release, waive, and discharge [Business Name], its owners, officers, employees, contractors, volunteers, agents, and property owners (together, "the Released Parties") from any and all claims, demands, damages, costs, and causes of action of any kind arising out of my participation in this activity, including those caused by the ordinary negligence of the Released Parties.

    How to use this zipline waiver

    Zipline operations are heavily engineered and lightly forgiving. The equipment rarely fails, but guest behaviour on the line — grabbing the cable, turning backwards, failing to brake — causes most injuries, and your waiver should name those behaviours specifically.

    Weight limits are a safety parameter, not a policy preference. Both the minimum and maximum matter: too light and a guest stalls mid-line, too heavy and braking distance changes. Make the participant confirm they are within range in writing.

    Include a rescue clause. Guests stall, freeze, or refuse to launch, and a staff-assisted rescue means physical contact and being lowered from height. Consent to that in advance avoids an awkward conversation at fifty feet.
