I am requesting to participate in equipment rental operated by [Business Name] ("the Company"). I understand that I will use the rented equipment without direct supervision by the Company, and that I am responsible for how, where, and by whom it is used during the rental period.
I confirm that my participation is entirely voluntary, that no one has pressured me to take part, and that I may stop participating at any time if I believe conditions are unsafe or beyond my ability.
I understand that equipment rental involves inherent risks that cannot be eliminated regardless of the care taken by the Company, and that those risks include, but are not limited to:

•Use of the equipment beyond the renter's skill, strength, or experience

•Serious bodily injury, permanent disability, paralysis, and death
I have read this list, I understand it, and I knowingly and voluntarily accept every one of these risks, including risks that may arise from the negligence of others or that are not specifically listed above.
I agree to follow all posted rules, safety briefings, and instructions given by [Business Name] staff, including:

I understand that failing to follow the rules may result in my removal from the activity without a refund.
I agree to inspect any equipment provided to me before use and to immediately report anything that appears damaged, worn, or unsafe. If I choose to use the equipment, I am confirming that I accept it in its current condition.
I accept responsibility for the reasonable cost of repair or replacement of equipment that is lost, stolen, or damaged beyond normal wear while it is in my care.
I confirm that I am not under the influence of alcohol, cannabis, illegal drugs, or any medication that impairs judgment, coordination, or reaction time, and I will not become so during the activity.
I understand the Company may refuse or end my participation, without refund, if staff believe I am impaired.
I confirm that I am physically able to take part in this activity and that I have disclosed any medical condition, injury, allergy, pregnancy, or medication that could affect my safety or the safety of others.

In the event of an injury or medical emergency, I authorize the Company to arrange first aid, emergency transport, and medical treatment on my behalf, and I accept financial responsibility for the cost of that care.

Rental waivers do a job that guided-activity waivers do not: the customer takes your property away and uses it without supervision. That single fact changes the whole document.
You need two things running side by side. A liability release for injury during use, and a rental agreement covering damage, loss, late return, and deposits. Operators who use a plain activity waiver end up with no contractual claim when a customer returns your equipment broken.
The unsupervised-use clause is the other essential. Once your equipment leaves the counter, you cannot control conditions, competence, or how many people use it. The waiver should say the customer accepts responsibility for use during the whole rental period.
