I am requesting to participate in birthday party activities operated by [Business Name] ("the Company"). I understand that party guests take part in group play and activities alongside children they may not know.

I understand that birthday party activities involves inherent risks that cannot be eliminated regardless of the care taken by the Company, and that those risks include, but are not limited to:

•Falls, collisions, and rough play during group games and free play

•Allergic reaction to cake, food, party favours, or decorations

•Injury on play equipment or attractions included in the party package

•Excitement-driven behaviour leading to unpredictable movement in the space

•Guests becoming separated from their responsible adult during the party

•Serious bodily injury, permanent disability, paralysis, and death

I have read this list, I understand it, and I knowingly and voluntarily accept every one of these risks, including risks that may arise from the negligence of others or that are not specifically listed above.

I agree to follow all posted rules, safety briefings, and instructions given by [Business Name] staff, including:

•Guests remain within the party area and any booked attractions

•All allergies and dietary restrictions disclosed before the party begins

•A responsible adult identified for each guest for the whole party

•Posted facility and attraction rules apply to party guests
