    2. Activity and Voluntary Participation

    I am requesting to participate in indoor playground and soft play activities operated by [Business Name] ("the Company"). I understand that children play at their own pace in a shared, actively used space alongside children of different ages and sizes.

    I confirm that my participation is entirely voluntary, that no one has pressured me to take part, and that I may stop participating at any time if I believe conditions are unsafe or beyond my ability.

    3. Assumption of Risk

    I understand that indoor playground and soft play activities involves inherent risks that cannot be eliminated regardless of the care taken by the Company, and that those risks include, but are not limited to:

    Falls from climbing structures, slides, and elevated platforms

    Collisions between children, particularly where ages and sizes are mixed

    Impact with padded and unpadded surfaces, netting, and support posts

    Entrapment or friction injury inside tubes, slides, and ball pits

    Exposure to communicable illness in a shared indoor play environment

    Allergic reaction to food or materials present in the facility

    Serious bodily injury, permanent disability, paralysis, and death

    4. Rules and Instructions

    I agree to follow all posted rules, safety briefings, and instructions given by [Business Name] staff, including:

    Grip socks worn at all times inside the play structure

    Observing posted age, height, and capacity limits for each area

    No food, drink, or hard toys inside the play equipment

    A responsible adult remaining on site for the duration of the visit

    Head first sliding, climbing up slides, and rough play are not permitted

    5. Health, Fitness, and Medical Authorization

    I confirm that I am physically able to take part in this activity and that I have disclosed any medical condition, injury, allergy, pregnancy, or medication that could affect my safety or the safety of others.

    6. Release of Liability and Indemnification

    How to use this indoor playground waiver

    Indoor playgrounds have a volume problem no other vertical shares. A Saturday morning can bring two hundred families through a single door, most of them repeat visitors, most of them arriving with more than one child. A form designed to be signed once per child per visit is the reason your queue is out the door at 10am.

    The clauses that matter for soft play are unglamorous: guardian authority, emergency medical consent, and an acknowledgement that staff supervise the space rather than individual children. That last point resolves most parent disputes before they start.

    Keep the annual renewal in mind while drafting. If your waiver is worded around "this visit", you cannot reuse it. Word it around a date range and returning families sign once a year.
