    2. Activity and Voluntary Participation

    I am requesting to participate in indoor climbing and bouldering operated by [Business Name] ("the Company"). I understand that climbing requires me to manage my own safety checks and that falling is a routine part of the activity.

    I confirm that my participation is entirely voluntary, that no one has pressured me to take part, and that I may stop participating at any time if I believe conditions are unsafe or beyond my ability.

    3. Assumption of Risk

    I understand that indoor climbing and bouldering involves inherent risks that cannot be eliminated regardless of the care taken by the Company, and that those risks include, but are not limited to:

    Ground falls while bouldering, including onto padded surfaces

    Ankle, wrist, knee, and spinal injury from uncontrolled landings

    Falling from height due to belay error, knot failure, or incorrect clipping

    Failing to attach to an auto-belay before leaving the ground

    Being struck by a falling climber, dropped equipment, or a swinging rope

    Rope burn, finger pulley injury, shoulder strain, and overuse injury

    Holds spinning, breaking, or loosening during a climb

    Serious bodily injury, permanent disability, paralysis, and death

    4. Rules and Instructions

    I agree to follow all posted rules, safety briefings, and instructions given by [Business Name] staff, including:

    Completing a partner check of harness, knot, and belay device before every climb

    Belaying only if you hold current certification from this facility

    Confirming attachment to the auto-belay before leaving the ground every time

    Keeping landing zones clear and not walking or sitting beneath climbers

    Minors climbing only under the supervision level required by the facility

    I understand that failing to follow the rules may result in my removal from the activity without a refund.

    5. Equipment

    I agree to inspect any equipment provided to me before use and to immediately report anything that appears damaged, worn, or unsafe. If I choose to use the equipment, I am confirming that I accept it in its current condition.

    I accept responsibility for the reasonable cost of repair or replacement of equipment that is lost, stolen, or damaged beyond normal wear while it is in my care.

    6. Health, Fitness, and Medical Authorization

    I confirm that I am physically able to take part in this activity and that I have disclosed any medical condition, injury, allergy, pregnancy, or medication that could affect my safety or the safety of others.

    How to use this climbing gym waiver

    Climbing gyms carry a risk nobody else has: the participant's own competence is a safety system. A climber who ties in incorrectly or belays badly endangers someone else, and your waiver should make the climber accountable for their own checks.

    Bouldering falls are the volume injury — ankles and wrists, over and over — and mats reduce but never remove that. Say so directly; a waiver implying padded floors make bouldering safe is worse than saying nothing.

    Auto-belays deserve a named clause too. Failing to clip in before climbing an auto-belay route is a recurring, well-documented cause of ground falls in commercial gyms.

    Before you use it

    • Make the climber responsible for their own knot, harness, and partner checks in writing.
    • Do not imply mats make bouldering safe — state that falls cause injury regardless.
    • Record belay certification alongside the waiver so staff can verify at the desk.
