I am requesting to participate in trampoline park activities operated by [Business Name] ("the Company"). I understand that jumping, flipping, and bouncing place high loads on the body and that landings are not always predictable.

I confirm that my participation is entirely voluntary, that no one has pressured me to take part, and that I may stop participating at any time if I believe conditions are unsafe or beyond my ability.

I understand that trampoline park activities involves inherent risks that cannot be eliminated regardless of the care taken by the Company, and that those risks include, but are not limited to:

•Double bouncing, where another jumper's rebound causes an uncontrolled landing

•Awkward or head-first landings in foam pits, on airbags, and on landing mats

•Ankle, knee, and lower-leg injury from repeated impact and rotation

•Attempting flips, twists, or tricks beyond the participant's ability

•Collisions with other jumpers, padding, frames, springs, and walls

•Neck and spinal injury, including permanent paralysis

•Serious bodily injury, permanent disability, paralysis, and death

I have read this list, I understand it, and I knowingly and voluntarily accept every one of these risks, including risks that may arise from the negligence of others or that are not specifically listed above.

I agree to follow all posted rules, safety briefings, and instructions given by [Business Name] staff, including:

•One jumper per trampoline at all times

•No double bouncing, wrestling, or intentional contact with other jumpers

•Foam pits and airbags entered feet first, one participant at a time

•Grip socks worn on all court surfaces

•Jumpers must clear the landing area before the next participant goes

•Court monitor instructions must be followed immediately

I understand that failing to follow the rules may result in my removal from the activity without a refund.

I confirm that I am not under the influence of alcohol, cannabis, illegal drugs, or any medication that impairs judgment, coordination, or reaction time, and I will not become so during the activity.

I understand the Company may refuse or end my participation, without refund, if staff believe I am impaired.

I confirm that I am physically able to take part in this activity and that I have disclosed any medical condition, injury, allergy, pregnancy, or medication that could affect my safety or the safety of others.

In the event of an injury or medical emergency, I authorize the Company to arrange first aid, emergency transport, and medical treatment on my behalf, and I accept financial responsibility for the cost of that care.
