I am requesting to participate in bicycle and e-bike rental operated by [Business Name] ("the Company"). I understand that I will ride on public roads, paths, and trails shared with vehicles, pedestrians, and other cyclists, without supervision by the Company.

I confirm that my participation is entirely voluntary, that no one has pressured me to take part, and that I may stop participating at any time if I believe conditions are unsafe or beyond my ability.

I understand that bicycle and e-bike rental involves inherent risks that cannot be eliminated regardless of the care taken by the Company, and that those risks include, but are not limited to:

•Collision with motor vehicles, pedestrians, other cyclists, and fixed objects

•Falls caused by road surface hazards, tram tracks, gravel, and wet conditions

•Head, facial, dental, wrist, and collarbone injury in a fall or collision

•Higher speeds, greater weight, and longer braking distance on e-bikes

•Mechanical failure of brakes, tyres, chain, or battery systems

•Unfamiliarity with local traffic laws, road layouts, and riding direction

•Theft of or damage to the bicycle while it is in the renter's care

•Serious bodily injury, permanent disability, paralysis, and death

I have read this list, I understand it, and I knowingly and voluntarily accept every one of these risks, including risks that may arise from the negligence of others or that are not specifically listed above.

I agree to follow all posted rules, safety briefings, and instructions given by [Business Name] staff, including:

•Obeying all traffic laws, signals, and local cycling regulations

•Wearing the helmet provided, and as required by local law for minors

•Using the supplied lock to secure the bike to a fixed object whenever unattended

•Using lights when riding at dusk, at night, or in poor visibility

•Reporting any crash, damage, or mechanical fault to the Company immediately

•Not exceeding the rated weight limit or carrying passengers on a single bike

I understand that failing to follow the rules may result in my removal from the activity without a refund.

I agree to inspect any equipment provided to me before use and to immediately report anything that appears damaged, worn, or unsafe. If I choose to use the equipment, I am confirming that I accept it in its current condition.

I accept responsibility for the reasonable cost of repair or replacement of equipment that is lost, stolen, or damaged beyond normal wear while it is in my care.

I confirm that I am not under the influence of alcohol, cannabis, illegal drugs, or any medication that impairs judgment, coordination, or reaction time, and I will not become so during the activity.

I understand the Company may refuse or end my participation, without refund, if staff believe I am impaired.

I confirm that I am physically able to take part in this activity and that I have disclosed any medical condition, injury, allergy, pregnancy, or medication that could affect my safety or the safety of others.
