I am requesting to participate in go kart racing operated by [Business Name] ("the Company"). I understand that karting involves driving a motorised vehicle at speed in close proximity to other karts on a shared circuit.

I confirm that my participation is entirely voluntary, that no one has pressured me to take part, and that I may stop participating at any time if I believe conditions are unsafe or beyond my ability.

I understand that go kart racing involves inherent risks that cannot be eliminated regardless of the care taken by the Company, and that those risks include, but are not limited to:

•Collisions with other karts, tyre barriers, walls, and track furniture

•Spins, rollovers, and sudden deceleration causing whiplash or bruising

•Burns from hot engine components, exhaust, and brake assemblies

•Entanglement of hair, clothing, scarves, or jewellery in moving parts

•Kart mechanical failure including steering, throttle, and brake systems

•Neck, back, and wrist strain from vibration and impact during the session

•Serious bodily injury, permanent disability, paralysis, and death

I have read this list, I understand it, and I knowingly and voluntarily accept every one of these risks, including risks that may arise from the negligence of others or that are not specifically listed above.

I agree to follow all posted rules, safety briefings, and instructions given by [Business Name] staff, including:

•Long hair tied back and secured; no scarves, loose clothing, or dangling jewellery

•Helmets and any provided head sock or neck brace worn correctly for the whole session

•Obeying all flag signals immediately, including the black flag

•No intentional bumping, blocking, or contact with other karts

•Remaining seated and belted until a marshal signals it is safe to exit

•Meeting the posted minimum height and age requirement for the kart class

I understand that failing to follow the rules may result in my removal from the activity without a refund.

I agree to inspect any equipment provided to me before use and to immediately report anything that appears damaged, worn, or unsafe. If I choose to use the equipment, I am confirming that I accept it in its current condition.

I accept responsibility for the reasonable cost of repair or replacement of equipment that is lost, stolen, or damaged beyond normal wear while it is in my care.

I confirm that I am not under the influence of alcohol, cannabis, illegal drugs, or any medication that impairs judgment, coordination, or reaction time, and I will not become so during the activity.

I understand the Company may refuse or end my participation, without refund, if staff believe I am impaired.

I confirm that I am physically able to take part in this activity and that I have disclosed any medical condition, injury, allergy, pregnancy, or medication that could affect my safety or the safety of others.
